When you get in touch, we’ll ask you the same questions from our online contact form. This will help us understand what you need help with.
We will use the information you give us to choose the most suitable doctor, nurse or health professional to help you.
Pharmacists can give you advice on a range of conditions and suggest medicines that can help. They may also be able to offer treatment and prescription medicine for some conditions, without you needing to see a GP. Read more about the Pharmacy First service and how pharmacists can help you.

Routine appointments and administrative requests
All routine appointments and administrative requests should be made via the online contact form called AccuRx. Clinical requests will be sent to the patient’s own GP for onward advice and action.
You can complete the AccuRx form in the following ways:
- use our appointment request form or via the NHS App, Monday to Friday. We will respond within 48 working hours
- phone us on 01664 503000, Monday to Friday, when we are open. The receptionist will be able to complete the form for you whilst they speak to you over the phone.
- visit the surgery, Monday to Friday, when we are open. There is a dedicated desktop computer available for patients to use to complete the online form whilst in the Practice. Reception staff are on hand to help if you need it.
Please share as much information as possible in the online forms, including your appointment preferences. We will use the information you give us to offer the most appropriate support for your needs.

Appointments by phone, video call or by text or email can be more flexible and often means the doctor or nurse can help you sooner.

We have the following extended hours:
Latham House Medical Practice
- Monday's: 7:30am to 8am
- Wednesday's to Friday's: 6:30pm to 8pm
Asfordby Branch Surgery
At our Asfordby Branch we are open Saturdays for blood and smear appointments from 9am to 5pm
If you need medical help now, use NHS 111 online or call 111.
NHS 111 online is for people aged 5 and over. Call 111 if you need help for a child under 5.
Call 999 in a medical or mental health emergency. This is when someone is seriously ill or injured and their life is at risk.
Please tell us:
- If there’s a specific doctor, nurse or other health professional you would prefer to respond.

- If you need an interpreter.
- If you have any other access or communication needs.
